Book a consultation with an OIT Expert at Kindercare

OPTION 1 - Ideally your child’s allergist or primary care provider (family doctor, nurse practitioner, pediatrician) faxes us a referral requesting a Food Allergy / OIT consultation. For their convenience, our referral form can be found here. Upon receipt of the referral request, we will contact you to schedule an appointment.

OPTION 2 - If your child does not have a primary care provider or allergist, you can schedule an appointment with our nurse practitioner who can help with an OHIP-insured assessment and, if appropriate, arrange for your consultation with Dr. Conway or Dr. Aujnarain.
